<p>DUTIES:<br />
The Nurse Practitioner (NP) with the East End Family Health Team (EEFHT) provides primary health care services working collaboratively with the interdisciplinary team. The Nurse Practitioner will perform comprehensive health assessments to diagnose, treat, prescribe, educate, and evaluate patient outcomes utilizing their full scope of practice. The Nurse Practitioner will fulfill their professional competencies: direct clinical practice, coaching of patients, families and other team members, consultant, research, lead, collaborate and make ethical decisions.</p>

<p>QUALIFICATIONS:<br />
• Certificate of competence as a Primary Health Care Nurse Practitioner with the College of Nurses of Ontario.<br />
• Current membership with the Registered Nurses Association of Ontario<br />
• Valid WHMIS review card<br />
• Two to five years of work experience preferred.<br />
• Current BCLS<br />
• Computer skills: Microsoft Office, Outlook, Excel and Word. Previous experience with an electronic medical record is an asset.<br />
• Excellent verbal and written communication skills.<br />
• Effective conflict resolution, problem solving and consensus building skills.<br />
• Must have a valid driver’s licence and access to a vehicle.<br />
• The ability to converse in English and French would be an asset.</p>

<p>This position provides nutrition and dietetic services to public health and Canada Prenatal Nutrition (CPNP) programs. The public health dietitian develops, implements and evaluates community based nutrition programs and services using a variety of health promotion strategies including awareness raising, education and skill building, environmental support and policy development. Based on identified needs, the public health dietitian works collaboratively with staff, coalitions, professionals, and agencies, in worksite, community and school settings to deliver nutrition services and programs.</p>
<p>Qualifications:</p>
<ul>
<li>Baccalaureate degree in foods and nutrition or equivalent program</li>
<li>Eligibility for membership with the College of Dietitians of Ontario will be required</li>
<li>Excellent oral and written communication skills</li>
<li>Fluency in both official languages considered an asset</li>
<li>Strong interpersonal and leadership skills</li>
<li>Self directed worker who requires minimal supervision</li>
<li>Valid driver’s license and use of a vehicle are required</li>
<li>Willingness to work flexible hours and travel to meet program needs</li>
