Engagement and Safety Liaison - TDH Offsite Facility - St. Mary's

Position Summary:Reporting to the manager the Rehab/CCC program, this position supports the day-to-day operations of TDH off-site facility at St. Mary’s Gardens by providing in-the-moment client-centered support for patients with responsive behaviors.  

Position Responsibilities: 

  • Support patients who have responsive behaviors related to dementia/mental health conditions.
  • Assist nursing in their response to patients exhibiting escalating behaviors.
  • Use conflict resolution skills to proactively address and deescalate potential crisis situations.
  • Build rapport with patients and provide patient-centered coaching and support.
  • Provide care based on the principles of personhood and GPA (gentle persuasive approach) in collaboration with a multidisciplinary team.
  • Utilize CPI techniques as required
  • Must be willing to partake in behaviour response training opportunities as they arise.
  • Must be able to work effectively and collaboratively as a team member.
  • Must have the ability to communicate effectively with people from a variety of backgrounds.
  • Must be organized and able to perform adequately under stress when confronted with unexpected and/or escalating situations involving aggression/violence
  • Perform other related activities, as necessary or as assigned.

Position Requirements: 

  • Current competency in CPR, CPI and GPA.
  • Learning ability to achieve and maintain organizational competencies.
  • Employment history must demonstrate regular attendance at work.
  • Must maintain privacy and confidentiality as per hospital policy.
  • Must possess recent paid or unpaid experience in working with clients with responsive behaviors
  • Must possess the ability to provide effective services to clients/patients in both English and French. Individuals who do not meet the language requirements of this posting are invited to submit their résumé for future consideration.
