Transitional Care Worker – Bilingual (English & French)

The Canadian Red Cross Society, a not for profit, humanitarian organization dedicated to helping Canadians, as well as the most vulnerable throughout the world, is currently seeking an experienced self-motivated individual for the position of Community Services Coordinator.
RESPONSIBILITIES
Coordinate, implement and evaluate the delivery of the Seniors’ Transportation Program within the defined communities.
Ensure excellent customer service and maintain positive relationships with other agencies and Red Cross departments.
Assess client needs and fees based on eligibility to access the program according to established policies and procedures, referring non-eligible persons to alternative services
Complete required documentation, creating and updating confidential files as necessary
Communicate pertinent program information to clients, workers/volunteers and other agencies
Maintain client registration list and follow up with clients in a timely and efficient manner
Ensure incidents/accidents are recorded and reported within the appropriate timeline
Develop implementation plans for program delivery
Respond to client inquires in a timely and professional manner
Follow up on client complaints adhering to the National complaint process
Complete program administration and provide statistical reports as required
Evaluate service delivery and present findings and recommendations for improvement
Adhere to provincial standards in the delivery of the program
Review, implement, and comply with all policies and procedures as directed
Entering relevant client information (both live and recorded) into the Client Information Management System (CIMS)
Verifying the integrity of data and making corrections as necessary
Providing client transportation service coordination, including developing, distributing client/volunteer service schedules and informing client/volunteer changes to schedule
Maintaining and coordinating the transportation schedule
Assisting with the volunteer recruitment process, as required within the Region
Receiving client account payments, course payments and donations, logging cheques, issuing receipts, balancing daily cash log
Responding to inquiries regarding programs and services offered by the Red Cross providing information and assistance
Directing callers to other community agencies or information sources where appropriate
Fleet Management - across the District (therefore must be able to travel throughout the District if required)
Assist with recruiting, training, coordination and evaluation of Volunteer Drivers
Performing other duties as required

QUALIFICATIONS
The minimum qualifications for this position are specialized post-secondary education in relevant human resource/community social services and between 3-6 years of job-related work experience, or an equivalent combination of education and experience.
Excellent computer skills in Microsoft Office software applications (i.e. MS Word, Excel, and MS Outlook)
Organizational skills and the ability to understand detailed information

Exceptional organizational skills, interpersonal and teaching skills
Ability to prioritize and handle multiple tasks
Ability to work non-traditional hours and travel as required
Bilingualism is a requirement (English and French)
WORKING CONDITIONS
The work is performed in an office environment that is clean and comfortable. Hours of service are based on 35-hour work weeks, Monday to Friday
Please note that there is no relocation package for this position.
