Strategic Clinical Partnership Opportunity
Blanche River Health invites expressions of interest from experienced Nurse Practitioners to provide advanced diabetes management services to the Timiskaming Diabetes Education Program (TDEP) through a structured virtual care model.
This engagement is designed to strengthen specialized diabetes care capacity across the Timiskaming District and advance Blanche River Health’s commitment to delivering Health Care Close to Home for rural and underserved communities.
The successful candidate will contribute expert clinical leadership in complex diabetes management while practicing autonomously within Nurse Practitioner scope.
Role Overview
The Nurse Practitioner will function as an independent contractor providing direct patient care and clinical consultation in collaboration with TDEP Registered Nurses and Registered Dietitians.
Clinical responsibilities will include:
· Comprehensive assessment and longitudinal management of complex Type 1 and Type 2 diabetes
· Initiation and titration of insulin therapies (basal, bolus, and combination regimens)
· Initiation and optimization of GLP-1 receptor agonists, oral therapies, and oral anti-hyperglycemic agents
· Ordering and interpretation of laboratory investigations relevant to diabetes management
· Interpretation of continuous glucose monitoring (CGM) data (Dexcom, Libre)
· Development of individualized titration plans and formal treatment orders
· Communication of care plans with primary care providers to ensure continuity and coordinated care
· All services will be delivered via secure, PHIPA-compliant virtual platforms supported by Blanche River Health.
Engagement Model
· Contract-based, fee-for-service structure
· Flexible scheduling within mutually agreed service volumes
· Independent prescribing and ordering authority within NP scope of practice
· Documentation in accordance with Blanche River Health policies and professional standards
· Collaborative partnership with the interdisciplinary Diabetes Education team
· Consultation can be via phone or Teams platform. Consultation requests will be coming from the TDEP team at Blanche River Health mainly for unattached patients.
Contractual Requirements
· Current registration in good standing with the College of Nurses of Ontario (NP-PHC or NP-Adult)
· Independent professional liability coverage (CNPS or equivalent) appropriate for independent contract practice
· Secure, private workspace with reliable high-speed internet and PHIPA-compliant technology
· Minimum 3–5 years’ demonstrated experience in advanced diabetes management, including complex insulin titration and cardiometabolic therapy optimization
· Has private or corporate insurance coverage if affiliated with a registered organization,
· Experience serving rural, Northern, or Indigenous populations considered an asset
