Outreach Worker - Community Withdrawal Management/Liver Health Team

The Outreach Worker will provide low threshold outreach to marginalized community members living with a substance use disorder. This position will provide people education, information, and the means to reduce the impacts of substance use. This includes referrals to relevant agencies, the distribution of harm reduction supplies and naloxone, and ongoing communication as needed.
Position Responsibilities:
- Active outreach in areas that are frequented by community members living with a substance use disorder.
- Deliver individual support using a harm reduction approach.
- Assist clients with needed paperwork such as applications for birth certificates, health cards or medical appointments.
- Assist clients to develop their capacities around treatment/medical scheduling and health management planning, taking into consideration the reality of the relevant social determinants of health for each individual.
- Routinely compile, enter and report confidential data at agency and funder’s request.
- Ensure that all necessary reports are completed and submitted to the appropriate sources, meeting set deadlines.
- Network with internal and community partners to ensure an optimal continuum of health & social services.
- Perform other related duties as assigned.
Position Qualifications:
- Post-secondary degree, diploma in addictions/social services or related field that is eligible for membership with the Ontario Association of Social Workers and Social Service Workers.
- Registered and in good standing with relevant college.
- Must be a member of the Ontario College of Certified Social Workers and Social Service Workers.
- Experience working in a community setting, preferably in the mental health and addictions field.
- Strong knowledge of issues affecting marginalized communities.
- Demonstrated experience dealing with clients who have poor distress tolerance and emotional regulation challenges.
- Knowledge of substance use, blood-borne illness, harm reduction, and models supporting people with lived and living experience.

Demonstrated knowledge of evidence-based practices of recovery principles and trauma informed care.
- Demonstrated knowledge of how to apply harm reduction approach to recovery and prevention of blood-borne illness.
- Understanding and knowledge of the recovery-oriented approach.
- Demonstrated ability to work with digital technology.
- Demonstrated critical thinking skills.
- Effective time management and ability to prioritize workload.
- Excellent interpersonal, presentation, and communication skills.

- Must have the ability to function as a member of a multi-disciplinary health care team.
- Must possess the physical ability to walk long distances, work while sitting; to bend, reach, lift and crouch.
- Ability to work some evening and/or weekend shifts
- Must be able to work in a variety of social and physical settings
- Facility in a second language is an asset.

Compensation and Benefits
- Starting salary of $28.69/hour; $55,945.50/year (As per Step 1 of the STW collective agreement.)
- Option to enroll in Healthcare of Ontario Pension Plan.
