Salary Range: $49,400 - $54,950
About the Role
The Patient Navigator, Community Outreach is a primarily community-based role within Misiway’s Integrated Care Team. This Navigator engages proactively with clients in their homes, in shelters, in drop-in spaces, and in community gathering places, building the long-term trust that is the foundation of effective health navigation.
The role focuses on two key populations: families facing barriers to care, including housing instability, food insecurity, and child welfare involvement; and seniors experiencing social isolation, frailty, or difficulty accessing primary care. By working upstream, this Navigator helps prevent avoidable crises and keeps clients meaningfully connected to the services and supports that keep them well.
This position is grounded in cultural safety, trauma-informed practice, and a deep respect for the self-determination of every client and family served.
What You’ll Do
Proactive Outreach and Engagement
• Conduct outreach to engage clients who may be disconnected from primary care or facing barriers to consistent engagement
• Perform wellness checks and follow-up visits in homes, shelters, drop-in programs, and community gathering spaces
• Build long-term trusting relationships as the foundation for sustained navigation and care engagement
• Identify clients at risk of care disengagement and intervene proactively to maintain connection
Families Facing Barriers to Care
• Support families experiencing housing instability, food insecurity, domestic stress, or child welfare involvement to access appropriate health and social supports
• Connect families with family-based programming, parenting supports, early childhood services, and wraparound resources in the community
• Work collaboratively with Children’s Aid Society, schools, and family service organizations when complex needs require multi-system coordination
Seniors Outreach
• Conduct regular wellness checks with seniors living alone or in social isolation
• Support seniors to access primary care, home support, chronic disease management programs, and social connection opportunities
• Identify signs of elder abuse, cognitive decline, or deteriorating health and escalate to the clinical team as appropriate
• Facilitate connections to falls prevention, caregiver support, and aging-in-place resources
Crisis Prevention and Response
• Recognize early indicators of health or social crisis and intervene proactively to prevent escalation and avoidable emergency visits
• Apply crisis awareness and de-escalation skills when clients are in acute distress
• Establish safety plans in collaboration with clients, families, and the care team when required
• Follow Misiway’s crisis response protocols and escalate to clinical staff when indicated
Documentation and Team Participation
• Maintain accurate and timely documentation of all client interactions and referrals in Misiway’s Electronic Medical Record (EMR)
• Participate in team meetings, case conferences, and care planning sessions
• Protect client privacy and confidentiality in accordance with Misiway’s Privacy Policies and PHIPA
What You Bring
Required
• Diploma in Social Services, Community Work, Health Sciences, or a related field, or an equivalent combination of relevant education and lived or community experience
• Minimum two (2) years of experience working with clients in a community-based, health, or social service setting
• Experience working in community settings with families and seniors experiencing complex social circumstances
• Familiarity with City of Timmins social services, including CDSSAB, Ontario Works, seniors’ programs, and family service agencies
• Crisis awareness and de-escalation skills
• Strong interpersonal, communication, and organizational skills
• Proficiency with computers; willingness to learn Misiway’s EMR system
• Valid Class G Ontario Driver’s Lisence and access to reliable transportation
• Understanding that this role carries mandatory reporting obligations under Ontario legislation, including the Child, Youth and Family Services Act; training will be provided by Misiway
Assets
• Experience working with Indigenous clients and communities, with demonstrated knowledge of Indigenous cultures, healing traditions, and the impacts of colonialism and intergenerational trauma
• Ability to recognize and respond appropriately to elder abuse and child welfare concerns
• Lived experience navigating health or social service systems as a patient, caregiver, or community member is recognized and valued as a meaningful qualification
• Ability to speak Cree or Ojibway
• Standard First Aid and CPR certification

Working Conditions
• Work is primarily community-based; regular travel within Timmins and surrounding areas is expected
• Standard hours are 35 hours per week, Monday to Friday
• Occasional flexibility in hours, including evenings or weekends, may be required to meet client and community needs
• This role involves exposure to complex social and health situations, including trauma, crisis, and grief; Misiway provides clinical supervision, debriefing support, and employee wellness resources
• This position includes a probationary period, during which a formal performance review will be conducted
